Sleep Screening Procedure
I hereby give consent to a free non-invasive sleep screening to determine my saturation levels, pulse rate and movement during sleep and that the report will be shared with the treating provider.
I take responsibility to return the device back to the pick up person or location within 1 business day from collection of the device.
